The power light is flashing. Using BPI+ enabled docsis 1.1. I see this line: "Network access is disabled by provisioning; no traffic will be forwarded."

Can you please guide me in getting this working?

5100 logs just look wrong to me, i can see what's wrong on a 5101 log just looking at the layout without reading
but 5100 log have a stupid order

THe last time i saw that on a 5100,CC was finishing out the D3 upgrades in my area. I believe that I had to ditch SBmod at that point...I'm pretty sure that was the case. It's tight here, I cannot use ANY flavor of Haxorware for more than an hour or so before the CMTS smacks it off the network. 5100's I can ONLY run stock firm with some telnet tweaks (NO, NOT Jr's magic trick!). I would say that your best move would be a fresh test setup with a new nic mac for starters...

Just for shits and giggles I would swap out the power supply on that 5100 just to make sure that is not the issue. They do die out eventually.

I remember this problem...

Try and replace the power supply...

But I think it is the modem is trying to do a firmware upgrade...
